Monthly Membership for Healthcare Services Information Sheet

We value all patients and are determined to be an affordable option for residents in and around Sevier County. We recognize the impact of increasing rise of commercial insurance premiums and the inability for residents to receive the healthcare they need. There are over 100,000 residents living in Sevier County and the majority work in the tourism industry. We understand the challenges of high-priced health insurance and want to provide an alternative option for healthcare services. 


We truly care about our patients and want to make a difference in each person's life without a significant impact to your personal finances. An Urgent Care visit in Sevier County is at least $125 for patients without insurance. Our monthly Membership fee could cost you less than one Urgent Care visit! Get the medical care you deserve at a price you can afford!!


Registration

·  Initial Membership Fee: $150 (per patient) – includes initial visit and basic labs (up to a 1-hour appointment)

  - Couples and Families: $200

      ($100 discount)


Monthly Fees

·  Individual - $100 per month & $20 per visit up to two visits per month

  - Billed on the 1st of the next month after date of initial visit


·  Couples - $150 per month & $20 per visit up to two visits per month per patient.

  - Billed on the 1st of the next month after date of initial visit

  - Must have the same address


·  Family - $150 per month & $20 per visit up to two visits per month per patient

  - Billed on the 1st day of the next month after date of initial visit

  - (Includes up to three family members – first child 18 - 22 included)

  - Additional Children – Age 18-22 $40 per month each


** $40 for each visit after the first two visits included in membership each month


Non-Membership Visit Fee


·   Uninsured patients: $80 per visit


What is a “Visit”?

A “visit” is defined as an in-person or telemedicine encounter with a provider to include but not limited to:

  - Medication refill – maximum of 2 refills per prescription or up to six months of medication

  - Sick visit

  - Well visit – routine care

  - Lab / radiology results review

  - Sports Physical

  - Annual exam

  - Telephone Consult lasting more than 10 minutes

FREQUENTLY ASKED QUESTIONS


1. Do I need health insurance to become a member?

  a.  No. Our Membership Program is an opportunity for those without insurance to have affordable access to a healthcare provider. If you do have commercial insurance, we recommend you verify with your insurance provider what could be covered under the Membership. Most commercial insurance companies do not reimburse for Membership programs. For patients without insurance, we recommend them carry catastrophic coverage for conditions that are not able to be addressed by our practice including specialty providers, comprehensive diagnostic studies, and emergency department visits.


2. Why do you offer this membership?

  a.  The amount of overhead required to work with insurance companies for reimbursement is unimaginable. We want to spend more time with our patients versus the typical 15-minute appointments that most providers offer in order to meet budget requirements. We recognize the shortage of affordable healthcare providers in Sevier County and have a genuine desire to help people regardless of their ability to afford commercial insurance.   


3.  Does my insurance pay for visits?

  a.  If you have commercial insurance, then yes. We will input your insurance information and bill them accordingly. You are not eligible to participate in the membership program due to restrictions with the insurance companies.

 

4.  Does my monthly membership fee or office visit fee count towards my insurance deductible?

  a.  No. You are not allowed to claim monthly membership fees against your deductible.


5.  Can I pay with Medicare / Medicaid?

  a.  No. And we will not bill Medicare or Medicaid. You are also not allowed to claim reimbursement for our services if you have Medicare or Medicaid. Legally, we are not allowed to include patients in our Membership program if you have Medicare, Medicaid, or TennCare.


6.  How will you collect payment for the membership?

  a.  We keep a check/credit card on file. The card will be charged the membership fees on the 1st of each month.


7.  Can I cancel my membership?

  a.  Yes. Membership can be cancelled at any time and will be effective on the last day of the current month. Keep the following other things in mind when cancelling:

     i.  There is no penalty to cancel.

     ii. Membership fees will not be reimbursed once processed.

     iii. Patients will be charged a re-enrollment fee of $150 to re-establish membership. The re-enrollment fee is to discourage abuse of our services.

      iv. Re-enrollment fees will not apply in cases where it would be unreasonable or unnecessarily punitive to apply them (for example, service members returning from deployment or active-duty service).

       v. We reserve the right to determine what is reasonable on a case-by-case basis.

      vi. We reserve the right to cancel your membership if the method of payment on file is denied by your bank.


8.  What is the registration fee?

  a.  The initial registration fee is $150 or $200 for all patients and families and is collected on the day the patient signs up. See the “membership fees” section.


9.  When do regular membership fees begin?

  a.  On the 1st day of the next calendar month regardless of registration date.


10.  What if I want to cancel after my registration fee is paid and I have my first visit?

  a.  The registration fee will not be refunded and will be treated as the fee for the office visit.

  b.  Once the registration or monthly fees are processed, they will not be refunded.

  c.  The initial registration fee is collected on the day the patient signs up.


11.  Do you offer one-time office visits for patients that do not want to join the membership program?

  a.  Yes. Each office visit is $80 for non-members or patients without insurance and is due prior to seeing the provider. 

  b.  Patients with commercial insurance will pay their regular co-pay if they have one.


12.  Is this type of membership only for wealthy people?

  a.  Actually, this type of membership is for all kinds of people. Our desire is to improve the health of the population in and around Sevier County that cannot afford commercial insurance. Typical insurance plans for Tennesseans can range from $159 (State of TN employee) for an individual to over $800 for a family. One report shows the average monthly premium for TN residents is between $537 - $864 a month.

13.  Are my membership fees tax deductible?

  a.  We recommend you discussing this with a tax professional.


14.  Will my membership fees increase?

  a.  We do not intend to increase the rates at this time. If the decision is made to increase the rate, an announcement on our website will be posted 90 days prior to the new fee taking place. See the Terms and Conditions for the Membership Program for more information.


15.  Do you see children?

  a.  Yes and no. We see children that have commercial, federal, or state insurance. We do not want to discourage families from seeing a traditional pediatric provider. We value their expertise, experience, and knowledge. Therefore, no children under the age of 18 will be allowed on the membership program.


16.  What if I sign up before the end of the month, pay the registration fee, but there are no appointment slots available?

  a.  We will address these on a case-by-case basis.

  b.  We will NOT take advantage of the situation nor inappropriately take your registration fee without being able to see you during that same month.

  c.   If we are truly fully booked and cannot see you until after the first of the next month, you will not be charged the monthly fee until the month after your initial visit.
